If you have ever had an MRI or X-ray that did not show anything, here is why ultrasound is different:
- What it is: A safe, radiation-free way to look deep into your muscles, tendons, and nerves using sound waves.
- The “Live Stream” Analogy: An MRI is like a still photograph, showing your joint frozen in time. An ultrasound is like a live stream, so we can watch your joint while you move and point to exactly where it hurts.

Why an MRI Is Not Always Enough
It is a common frustration: you have real pain, but the MRI comes back normal. This happens because MRIs are usually done while you are lying perfectly still. At ROSM, we use Dynamic Ultrasound. We can scan your shoulder while you move your arm, or your ankle while you flex your foot. By seeing your body in action, we can often spot the impingement, tear, or nerve issue that only appears when you move.

The Advantages of the Live View
- No Radiation: Unlike X-rays or CT scans, ultrasound is completely safe and uses no harmful radiation.
- Side-by-Side Comparison: We can quickly scan your good knee and compare it to your injured knee in real time to see exactly what has changed.
- Immediate Results: You do not have to wait a week for a report. Your doctor looks at the screen with you and explains what they see immediately.
The “Truth Test”: Diagnostic Injections
Sometimes, an image shows several things that could be causing pain. To be sure before we start a major treatment like PRP or BMAC, we may perform a diagnostic injection. Using ultrasound guidance, we inject a tiny amount of numbing medicine into the specific spot we think is the problem. If your pain disappears instantly, we have found the source, and we never waste time treating the wrong area.
The Journey: What to Expect
- Step 1, The Setup: You will be positioned comfortably. No tunnel or loud clicking noises like an MRI.
- Step 2, The Gel: We apply a clear, water-based gel to your skin. It is non-toxic and will not stain your clothes.
- Step 3, The Scan: We move a small handheld wand over the area and will likely ask you to move your limb so we can watch the tissue react.
- Step 4, The Plan: Once the scan is done, usually 5 to 10 minutes, your doctor will discuss the findings with you right then and there.
A note on cost: Musculoskeletal ultrasound is billed as a diagnostic exam, so many insurance plans cover it differently than they cover regenerative treatments. Coverage still varies by plan, and HSA or FSA funds can typically be used for any out of pocket cost.
